The award is bestowed on a physician in recognition for long standing and significant contribution to the diabetes community. The award consists of a personal plaque for the recipient and reimbursement for travel and accommodation expenses to attend the presentation ceremony at the Diabetes Canada/CSEM Professional Conference and Annual Meetings each year.
A nominee must:
- Be a physician;
- Have demonstrated long standing and dedicated service to the diabetes community; and,
- Have performed service in the form of clinical care, teaching, advocacy, research or a combination of these
A nomination must:
- Be made by two members of the Professional Section of Diabetes Canada; and,
- Include a complete nomination form and a detailed description of the nature and extent of the service and contribution which the candidate has rendered to the diabetes community.
Nominations are reviewed and selected by a Selection Committee comprised of the Professional Section National Executive of Diabetes Canada.
Deadline for nominations
All nominations must be recieved by April 17, 2020 and will stand for three years.
