Susie Jin will guide community pharmacists through a webinar to help them work with their patients living with type 2 diabetes.
Learning Objectives:
By the end of this 1-hour webinar, participants will be able to support diabetes management in adults with type 2 diabetes by:
- Optimizing pharmacotherapy for people with underlying cardiorenal conditions
- Matching pharmacotherapy to the person’s priorities of care such as “frailty", weight management and minimizing the risk of hypoglycemia
- Effectively communicating with the person’s inter-professional diabetes health-care team
- Learning and applying Diabetes Canada's tools for enhanced decision-support
