Diabetes Core Neighbourhood Program

The Diabetes Risk Assessment and Education Program is a project designed to increase diabetes awareness, assess diabetes risk factors, and facilitate early diagnosis within diverse inner-city communities. Early diagnosis and education can lead to better diabetes management which can decrease or delay the development of the common complications of diabetes.

Program components include: CanRisk Diabetes Risk Assessments, Learning Series Presentations, Presentations by Healthcare Professionals and an Inner City Healthy Living Expo.
