Implement health coaching approaches at your health care practice to support at-risk 'pre-diabetes' populations, or people living with type 1 or type 2 diabetes mellitus, to self-manage their care. Health coaching is an evidence-based strategy that can help health care providers manage the chronic conditions of their patients.

This 30-hour skills-training certificate program will focus on the implementation of diabetes health coaching approaches in primary care, community health centres, or specialized clinics. The pre-conference program includes three online learning modules, and a live session featuring practice simulations that will be held in Halifax prior to the 2018 Diabetes Canada conference.

Start date (online): September 18, 2018
Pre-conference in-class session: October 9, 2018 from 10 AM – 4 PM
